Substance Use Disorder Counselor

Job Description:

  •    Provide substance abuse counseling to clients requiring assistance dealing with substance mis-use, such as opioid use disorder, alcohol use disorder, and stimulant misuse.
  • Provides assessment, supportive SUD counseling and case management to clients in an outpatient setting. Conducts intake of new clients in compliance with regulatory standards.
  • Provides direct services to clients and completes all required clinical documentation in accordance with organization requirements.
  • Provide a safe and therapeutic environment by monitoring patients and referring to more intensive behavioral health services when needed.
  • Develops effective multidisciplinary treatment plans for patients. Assumes case management responsibility for assigned patients.
  • Assist patients in initiating medication assisted treatment in coordination with patient's primary medical provider and care team.
  • Act as client advocate in order to coordinate required services or to resolve urgent medication issues.
  • Act as a single point of contact to receive referrals, from medical providers and behavioral health clinicians
  • Support patient engagement, linkage, coordination, monitoring, follow-up and advocacy
  • Maintain close communication with related health, educational, behavioral health and social service agencies
  • Promote timely and coordinated internal communications among patients, clinic primary care providers, behavioral health clinicians, case managers, consulting/treating psychiatrists and others involved in the patient's care provision
  • Facilitate warm hand offs between medical and behavioral health staff and provide referrals for community resources to support whole person health.


  • Cultural competence working in diverse/low income and immigrant communities.
  • Knowledgeable in the principles, practices and techniques of social work; basic psychological theory and practices as related to human behavior
  • Familiarity with medication assisted treatment (MAT) approaches
  • Familiarity with Motivational Interviewing required.

Certifications / Requirements:

  • Bachelor's Degree from an accredited program or university preferred
  • Drug and Alcohol Certification recognized by Department of Health Care Services (DHCS).
  • Bilingual in Spanish and English Required
  • Relevant experience and/or training in residential chemical dependency treatment preferred
  • Experience providing support and advocacy
  • If in recovery, three years of sobriety required.
