Transitional Housing Program Coordinator

(VIP Coordinator, ADVANCE Coordinator, SILS Coordinator)

The Program Coordinator for Transitional Housing Programs reports directly to the Director of Behavioral Health.  The Program Coordinator is responsible for the delivery of comprehensive clinical-based case management services to homeless veterans.  The main function of these programs is to assist homeless veterans in transitioning to permanent housing.


  • Provide guidance, direction and supervision to case management (CM) staff
  • Assist in facilitating clinical meeting in Director of Behavioral Health absence
  • Reviews client files for quality assurance, including discharged charts
  • Leads discussions on how to approach particular situations regarding client progress
  • Provides CM services to assigned caseload in any of U.S.VETS programs, as needed
  • Solicits outside providers to facilitate program classes for residents with approval by Director of Behavioral Health
  • Creates program class schedule/calendar and turns in to Director of Behavioral Health for approval
  • Collaborates with VSC to provide clinical guidance to VSA staff
  • Facilitates and attends roll calls and town halls, as needed
  • Acts as a liaison between CM staff and operations staff in making requests for supportive services
  • Creates CM schedule and ensures CM schedule covers a late night
  • Covers Case Manager responsibilities in their absence
  • Case management coverage provided as needed
  • Responsible for reporting client information to Director of Behavioral Health on various aspects of the program
  • Audits all files a minimum of quarterly according to U.S.VETS standards
  • Monitors program fee (rent) collection
  • Participates in client’s transition meetings
  • Responsible for training new Case Managers in all aspects of CM responsibilities
  • Collaborates with the Director of Behavioral Health and VA for entry/re-entry of clients
  • Request random UA's weekly for residents or as needed
  • Facilitates weekly program classes, as needed
  • Assists in monitoring of failed room inspections
  • Addresses documented in Incident Report
  • Assists with pass approval
  • Assists with outside referral sources as needed
  • Supervises data entry and updating of the Homeless Management Information Systems and responsible for accuracy of data
  • Performs other duties as required


  • Bachelor’s Degree in social services field or other related field required. Master's Degree preferred.
  • Minimum of two years of supervisory experience in related field.  Two years of employment at U.S.VETS may substitute for supervisory experience.
  • Registration with the BBS or registration/certification as a Substance Abuse Counselor preferred.
  • Experience working with homeless and/or veteran population preferred.
  • Ability to work effectively with a diverse group of clients, staff, and community members.
  • Excellent written and oral communication skills.
  • Leadership and conflict management skills.
  • Demonstration of personal and financial integrity in the workplace.
  • Ability to take direction, work independently with minimal oversight, and to work within a team.
  • Computer proficient in Microsoft Office.
  • Valid driver’s license required.  Must meet company insurance requirements and complete a provided driver training course.

Certifications / Requirements:

  • Bachelor's Degree required
  • Master's Degree preferred
  • BBS registration or registration/certification as a Substance Abuse Counselor
  • Minimum of 2 years of supervisory experience
